AIMS: Increasing social concern surrounds the potential adverse health effects of precarious employment (PE). In this study, we explored the association between PE and the onset of depressive symptoms. METHODS: A total of 11,555 Korean waged workers (5700 females) contributed 62,217 observations from 2009 to 2022. PE was operationalized as a multidimensional construct, including employment insecurity, income inadequacy and lack of rights and protection. Depressive symptoms were evaluated using the Center for Epidemiological Studies-Depression Scale (11-item version). The association between PE and the onset of depressive symptoms in the subsequent year was estimated using generalized estimating equations. Effect sizes were reported as odds ratio (OR) and 95% confidence interval (CI). RESULTS: The overall incidence of depressive symptoms was 8.3% during the study period. In cross-sectional analysis, daily employment, disguised employment, lower monthly wages and lack of social insurance coverage were associated with concurrent depressive symptoms in both men and women. Longitudinally, fixed-term employment (OR: 1.17, 95% CI: 1.07-1.29), daily employment (OR: 1.64, 95% CI: 1.45-1.85) and disguised employment (OR: 1.36, 95% CI: 1.17-1.57) were associated with the onset of depressive symptoms among the overall sample. Among men, the lowest quartiles of wage were associated with the onset of depressive symptoms (OR: 1.34, 95% CI: 1.13-1.60), while the absence of a trade union was associated among women (OR: 1.18, 95% CI: 1.01-1.39). CONCLUSIONS: Employment insecurity, inadequate income and lack of rights and protection may contribute to depressive symptoms. Therefore, PE serves as a significant social determinant of mental health among workers in Korea. Active policy efforts are warranted to improve the overall quality of employment in the workforce.

The global shipping industry, responsible for delivering over 70% of the world's goods (in volume), has increasingly adopted cost minimization policies, contributing to precarious employment practices that adversely affect seafarers' wellbeing. This study focuses on the intricate relationship between employment precarity and food provision on cargo ships. By presenting seafarers' perspectives, we aim to understand how precarious employment practices and cost minimization in the industry influence power dynamics related to food and impact seafarers' wellbeing. Drawing on empirical data collected through shipboard observations and interviews with seafarers, this study examines the often-overlooked experiences and perspectives of seafarers. The research sheds light on the precarity of employment in shipping and its inherent impact on the provision of food on board and its implications for seafarers' physical and emotional health, including the availability of nutritious and sufficient food and its impact on their daily lives. Through in-depth interviews, seafarers' insights into their experiences of food including the quality, availability, and cultural appropriateness of food on board are explored, as well as the standard of training for cooks. Through this research, we found substandard conditions on some of the ships, cost-focused decision-making, and lack of standardized food preparation practices on board. These findings underline the need for improved regulations, better training opportunities, and increased consideration for seafarers' wellbeing. These changes are essential to ensure the provision of adequate and nutritious meals that promote the physical and mental health of seafarers on board ships. Specifically, the research underscores the need for policy and advocacy initiatives to improve seafarers' lives and promote fair working conditions in the global shipping industry. By amplifying the voices of seafarers and providing evidence-based insights, this study contributes to the larger discourse on workers' rights and the importance of decent working conditions. It calls for greater attention to the provision of adequate, nutritious, and culturally appropriate food on board cargo ships, recognizing its significance for seafarers' physical and mental wellbeing, as well as a call for standardized training for ship's cooks.

The prevalence and severity of smoking are particularly high in populations in precarious situations and make smoking an essential determinant of social inequalities in health, due to its particularly catastrophic impact on the health of these populations. The general reduction in smoking, less significant in disadvantaged populations, contributes to further increasing these inequalities, and smoking tends to be concentrated in the most vulnerable populations. The relationships between tobacco and precariousness are examined by identifying, based on a review of the literature, the main common factors of vulnerability: stress and social adversity, self-stigma, a low feeling of self-efficacy, the social function of tobacco use, the ambivalence of demands, associated addictions, the severity of tobacco dependence, distance from support and care systems, etc. Specific attention is paid to certain particular conditions: mental health disorders, addictions (other than tobacco), inadequate housing, detention, migration. On this basis, courses of action are proposed to improve access to care and its effectiveness for the people concerned. Emphasis is placed on the mobilization of professionals who often tend to neglect issues of smoking in view of the immediate severity of the problems at the origin of the demands of the people received. A support offer for harm reduction (vaping in particular) seems particularly suited to often-ambivalent demands. The importance of networking and the involvement of CSAPAs/CAARUDs is also underlined. At the systemic level, all of this must be accompanied by political advocacy to reduce these inequalities and social determinants of health.


La prévalence et la sévérité du tabagisme sont particulièrement élevées dans les populations en situation de précarité et font du tabagisme un déterminant essentiel des inégalités sociales de santé, du fait de son impact particulièrement catastrophique sur la santé de ces populations. La réduction générale du tabagisme, moins importante dans les populations défavorisées, contribue à accroître encore ces inégalités et le tabagisme a tendance à se concentrer dans les populations les plus vulnérables. Les relations entre tabac et précarité sont examinées en identifiant, sur la base d'une revue de la littérature, les principaux facteurs communs de vulnérabilité : stress et adversité sociale, auto-stigmatisation, faible sentiment d'auto-efficacité, fonction sociale du tabagisme, ambivalence de la demande, addictions associées, sévérité de la dépendance au tabac, éloignement des dispositifs d'aide et de soins… Une attention spécifique est prêtée à certaines conditions particulières : troubles mentaux, addictions (hors-tabac), mal-logement, détention, migrations. Sur cette base, des pistes d'actions sont proposées pour améliorer l'accès aux soins et leur efficacité chez les personnes concernées. Un accent particulier est porté sur la mobilisation des professionnels qui ont souvent tendance à négliger les questions de tabagisme au regard de la sévérité immédiate des problèmes à l'origine de la demande des personnes accueillies. Une offre d'accompagnement à la réduction des risques (vapotage notamment) semble particulièrement adaptée à la demande souvent ambivalente des personnes. L'importance du travail en réseau et de l'implication des CSAPA/CAARUD est également soulignée. Sur le plan systémique, tout ceci doit s'accompagner d'un plaidoyer politique pour réduire ces inégalités et déterminants sociaux de santé.

In 2020, food shortages occurred at the beginning of the confinement period that was supposed to curb the COVID-19 pandemic. In French Guiana, where a major part of the population lives under the poverty line, health workers voiced major concerns. Alongside massive food aid distributions, a first transversal study was carried out in August 2020 targeting poor neighborhoods in Cayenne. The results were particularly worrying. More than 80% of households had been suffering from hunger during that month, with a median decrease of 46% in revenue. Two other investigations followed in February and then in August of 2021. With the relaxing of the health measures, the situation improved in the Cayenne region, but two out of three were still affected, showing signs of quantitative deficiencies and insufficient food diversity. The situation seemed particularly grave for children. In light of this situation, we propose to create an observatory of food insecurity in Guiana, while maintaining this topic as a health priority. In addition, the fight against food insecurity cannot be limited to multi-sectorial material and strategic aide. It must be thought about in a more global manner, including health and social questions, territorial management policies, access to land and water, access to rights and social inclusion. Targeted actions helping the most exposed and vulnerable people is also an important stake, independent of the administrative situation and residency rights of the concerned people.


En 2020, des pénuries alimentaires sont survenues dès l'entrée en vigueur du confinement destiné à contrôler la pandémie de COVID-19. En Guyane Française, où une forte proportion de la population vivait déjà sous le seuil de pauvreté, des alertes préoccupantes ont émané d'acteurs de santé. En parallèle du déploiement d'une aide alimentaire massive, une première enquête transversale a été menée en août 2020, ciblant les quartiers précaires des environs de Cayenne. Les résultats étaient particulièrement inquiétants : plus de 80 % des ménages avaient souffert de la faim dans le mois, avec une baisse médiane de revenus de 46 %. Deux autres enquêtes ont suivi, en février, puis en août 2021. Avec l'allègement des mesures sanitaires, la situation s'était sensiblement améliorée dans la région de Cayenne, mais deux ménages sur trois restaient impactés, avec des carences quantitatives et une diversité alimentaire insuffisante. La situation semblait particulièrement critique parmi les enfants. Au vu de cette situation, nous proposons de créer un observatoire de l'insécurité alimentaire en Guyane, tout en maintenant ce sujet en tête des priorités sanitaires. En outre, la lutte contre l'insécurité alimentaire ne peut se limiter à l'aide matérielle : la stratégie, multisectorielle, doit être pensée en globalité, intégrant les problématiques sanitaires et sociales, les enjeux de l'aménagement du territoire, de l'accès à la terre et à l'eau, de l'accès aux droits et à l'insertion sociale. Un ciblage juste des actions vers les publics les plus exposés et vulnérables est également un enjeu important, indépendamment de la situation administrative et du droit au séjour des personnes concernées.

O estudo apresentado neste artigo investigou o contexto em que se dá o trabalho de entregadores via aplicativos, levando em consideração problemas agravados pela pandemia de covid-19, tais como: o fato de ser realizado na rua, o sucateamento de direitos trabalhistas e sua precarização com o fenômeno de uberização. Os resultados apontam impactos negativos na saúde biopsicossocial desses entregadores, evidenciando uma queda da remuneração e uma maior exposição ao vírus. Dentre os impactos, destacam-se o aumento de processos psicopatológicos, além do medo em relação à iminente possibilidade de contrair o coronavírus e contaminar sua rede de apoio social. Desta maneira, conclui-se que a pandemia de covid-19, aliada a políticas de sucateamento dos direitos trabalhistas influenciam diretamente a saúde biopsicossocial desses trabalhadores, empurrando-os para a margem do trabalho formal e potencializando o processo de uberização relacionado a esse modelo de trabalho informal


The study presented in this article examined the work context of delivery drivers via apps, taking into account the problems aggravated by the covid-19 pandemic such as: the fact that the work is carried out on the street, the scrapping of labour rights and the precariousness of their work, due to the phenomenon of uberization. The results reveal negative impacts on the biopsychosocial health of these delivery drivers, highlighting a drop in remuneration and a greater exposure to the virus. Among these impacts, the increase in psychopathological processes as well as the fear of the imminent possibility of contracting the corona-virus and contaminating their social support network are the most visible. In this way, it is concluded that the covid-19 pandemic combined with policies to scrap labour rights have influence on the biopsychosocial health of these workers, pushing them to the margins of formal employement and intensifying the uberiza-tion process related to this informal work model


L'étude presentée dans cet article a examiné le contexte dans lequel des livreurs par l'entremise d'appli-cations travaillent, en tenant compte des problèmes aggravés par la pandémie de covid-19, tels que: le fait que le travail soit effectué dans la rue, la supression des droits du travail et sa précarisation causé par le phénomène d'ubérisation. Les résultats indiquent des impacts négatifs sur la santé biopsychosociale de ces livreurs, montrant une baisse des rémunérations et une plus grande exposition au virus. Parmi ces impacts, attirent l'attention l'augmentation des processus psychopathologiques, de la même manière que la crainte de la possibilité imminente de contracter le coronavirus et de contaminer leur réseau de soutien social. Il est conclu que la pandémie de covid-19 combinée aux politiques de suppression des droits du travail influent directement sur la santé biopsychosociale de ces travailleurs, en les poussant en marge du travail formel et en renforçant le processus d'ubérisation lié à ce modèle de travail informel

BACKGROUND: Precarious employment (PE) has been conceptualized as having a multidimensional nature, and research addressing this topic in Asian countries is scarce. This study examined the typologies of PE and their relationships with mental health among Korean workers. METHODS: From October 2020 to April 2021, the Korean Working Conditions Survey was conducted on Korean wageworkers (weighted N = 38,347), representative of the nation. With theory-based operationalization of PE, we identified typologies and classified workers through latent class analysis. Logistic regression was performed to explore the association between PE typologies and mental health among workers, represented as odds ratio (ORs) and confidence intervals (CIs). RESULTS: Four distinct employment typologies emerged: (i) standard employment relationships (SER, 34.5%), (ii) moderate employment precariousness (MEP, 43.5%), (iii) underemployment (17.9%), and (iv) daily laborers (4.1%). Women were overrepresented in the MEP and underemployment types, while the SER type consisted of those with higher educational attainment and white-collar jobs. The OR (95% CI) of depression was 1.38 (1.27-1.50) for the MEP type, 1.48 (1.32-1.66) for the underemployment type, and 2.15 (1.85-2.51) for the daily laborers, compared to the SER type. For individual elements of PE, temporary employment, low wages, low-income predictability, and low involvement in work process were positively associated with depression, whereas part-time employment was negatively associated with depression. CONCLUSION: Employment insecurity, low material rewards, and a lack of rights and protection can contribute to the poor mental health of workers. Policy interventions are warranted to mitigate inequalities in employment quality among Korean workers.

OBJECTIVE: The primary objective was to determine the profile of patients consulting in an emergency department and diagnosed with a pelvic cancer. Our secondary objective was to assess the potential impact on this diagnostic trajectory on survival. METHOD: A single-center retrospective study including patients managed for a pelvic cancer between January 2018 and November 2020 in the center Hospitalier Intercommunal de Creteil was conducted. Patients' characteristics were compared based on their diagnostic trajectory (emergency or referred to consultation). Precariousness was assessed using Pascal's tool based on 4 characteristics: being a beneficiary of the former Couverture Maladie Universelle (CMU) or Aide Medicale d'Etat (AME), not having complementary health insurance, being job seeking for more than 6 months and being beneficiary of allowances. A patient was defined as precarious if the Pascal tool was 'TRUE', i.e., at least one positive item. The main socio-demographic and cancer associated factors were analyzed as prognostic factors. RESULTS: Over the inclusion period, among the 283 eligible patients, 37.3 % (87/233) had a diagnosis of cancer following an emergency department visit. There was a significant association between precariousness, rupture of gynecological follow-up, lack of participation in national screening campaigns and the risk of being diagnosed through the emergency pathway for all cancers studied (p = 0.001). There was no difference in terms of stage at diagnostic, management (according to current guidelines), prognostic and overall survival between the two groups. CONCLUSION: Patients in a situation of precariousness are more likely to be diagnosed with cancer in an emergency department. Our study underlines the importance of precariousness as a factor determining the type of diagnostic management of gynecological cancer. Efforts should be made toward improving frail patients to primary care.

A Psicologia Social do Trabalho tem contribuído para a visibilidade e ampliação da compreensão do trabalho em seus diferentes formatos e contextos de atividade, alertando para a necessidade de se olhar para os trabalhos não regulamentados, como é o caso da atleta profissional. Nesse contexto, ainda mais fragilizada é a relação de trabalho da mulher atleta, principalmente, quando ela decide ser mãe e a precariedade no trabalho se acentua. Este artigo examina a trajetória de uma atleta de rendimento da seleção brasileira de vôlei, Tandara Caixeta, por meio do que tem sido veiculado nas mídias eletrônicas, especialmente os sites esportivos, sobre sua relação trabalhista com um clube. Foram analisadas 31 reportagens em sites que relacionavam com a gravidez e sua busca na justiça por seus direitos trabalhistas. A partir dos materiais coletados foi possível compreender que a problemática em questão versou sobre dois principais pontos: a relação contratual e a desproteção das atletas por ocasião da maternidade. Conclui-se que o apoio social, familiar e da equipe é fundamental para um retorno saudável da atleta ao seu rendimento, mas o apoio financeiro é fundamental para a manutenção do bem-estar e satisfação laboral. Social Work Psychology has contributed to enhancing the visibility and understanding of work across its various formats and activity contexts, emphasizing the importance of considering unregulated forms of labor, as seen in the case of professional female athletes. Within this context, the working relationship of female athletes becomes even more fragile, particularly when they decide to become mothers, amplifying the challenges within their working arrangements. This article examines the journey of a high-performance athlete from the Brazilian volleyball team, Tandara Caixeta, based on her electronic media coverage, particularly on sports websites, regarding her employment relationship with a club. Thirty-one reports related to pregnancy and her pursuit of labor rights through legal action were analyzed. The collected materials enabled us to discern two primary issues: the contractual relationship and the lack of protection for athletes during maternity. The study concludes that social, family, and team support are crucial for a healthy return of the athlete to her performance, but financial support is essential for maintaining overall well-being and job satisfaction. La Psicología del Trabajo Social ha contribuido para la visibilidad y amplia-ción de la comprensión del trabajo en sus diferentes formatos y contextos de actividad, alertando sobre la necesidad de mirar el trabajo no regulado, como es el caso del deportista profesional. En este contexto, la relación laboral de la deportista es aún más frágil, especialmente cuando decide ser madre y se acentúa la precariedad del trabajo. Este artículo propone seguir la trayectoria de una deportista de élite de la selección brasileña de voleibol, Tandara Caixeta, a través de lo publicado en medios electrónicos, especialmente sitios web depor-tivos, sobre su relación laboral con su club. Se analizaron 31 denúncias en sitios web relacionados con el embarazo, que reclamaron sus derechos laborales en los tribunales. A partir de los materiales recolectados se pudo entender que el tema en cuestión trataba sobre dos puntos principales: la relación contractual y la desprotección de los deportistas durante la maternidad. Se concluye que el apoyo social, familiar y de equipo es fundamental para un retorno saludable de la deportista a su desempeño, pero el apoyo económico es fundamental para mantener el bienestar y la satisfacción laboral. Resumo O artigo analisou o processo de precarização das relações de trabalho no setor da saúde, demonstrando como essa situação gerou sofrimento dos trabalhadores. O texto buscou registrar como a adoção institucional das concepções econômicas neoliberais, de redução das políticas sociais de Estado, acopladas a um histórico de rejeição cultural aos direitos trabalhistas, proporcionou a atração dos mecanismos de precarização do trabalho, notadamente, a terceirização da mão de obra e da prestação de serviços. No segundo momento, o texto aponta as impropriedades jurídicas cometidas, apresentando as consequências da fragilização do vínculo jurídico nas condições de trabalho desses trabalhadores, principalmente no que se refere às jornadas de trabalho e a remuneração, destacando-se, também, o quanto a disseminação desses vínculos representou um fator de rebaixamento geral dos salários e direitos trabalhistas e previdenciários no setor. Foram apresentados, ainda, dados das pesquisas da Fiocruz sobre condições de trabalho durante a pandemia. Conclui mostrando a urgência de se resgatarem os institutos jurídicos trabalhistas que foram abandonados no percurso histórico estudado, para direcionar a novo rumo as políticas públicas na saúde.


Abstract The present article analyzed the process of precarious work relations in the health sector, demonstrating how this situation generated illness and suffering among workers. The text sought to register how the institutional adoption of neoliberal economic conceptions, of the reduction of State social policies, coupled with a history of cultural rejection of labor rights, provided the attraction of mechanisms of precarious work for the public sector, notably, the outsourcing of labor and the provision of services. In a second moment, the text points out the legal improprieties committed, presenting the consequences of the weakening of the legal bond in the working conditions of these workers, mainly regarding working hours and remuneration, also highlighting how much the dissemination of these links represented a factor of a general reduction in wages and labor and social security rights in the sector. Data from Fiocruz surveys on working conditions during the pandemic were also presented. The article concludes by showing the urgency of rescuing the legal labor institutes that were abandoned in the historical path studied in order to guide the public health policies in a new direction.

BACKGROUND: Physical abuse often begins at a very young age and sometimes results in serious or fatal injuries. It is crucial to diagnose physical abuse as early as possible to protect this vulnerable population. OBJECTIVE: To study the factors associated with the first hospitalization for physical abuse from birth to the infant's first birthday in France. PARTICIPANTS AND SETTING: We included all singleton children born in a hospital setting in France between 2009 and 2013, who were identified from the French national information system database (SNDS). METHODS: To study factors associated with the first hospitalization for physical abuse during the first year after birth, we used the Fine and Gray regression model. Factors included in the multivariate model were the infant's sex, prematurity, neonatal conditions, the number of hospitalizations (at least two), medical consultations and complementary universal health insurance (proxy for family precariousness and socio-economic vulnerability). RESULTS: Over the 2009-2013 period, among 3,432,921 newborn singletons, 903 (0.026 %) were hospitalized for physical abuse in the year following birth. Among the factors associated with physical abuse, such as prematurity (aHR = 2.2[1.8-2.7]), male sex (aHR = 1.3[1.2-1.5]), or having had at least two hospitalizations (aHR = 1.7[1.4-2.1]), we found that complementary universal health insurance coverage was the factor most associated (aHR = 4.1[3.5-4.7]) with being hospitalized for physical abuse. CONCLUSION: These findings could help introduce preventative measures for infant protection in certain groups, such as parents of infants born prematurely, especially if they are in a precarious situation. This study also suggests that particular attention should be paid to infants who have been hospitalized at least two times in the first year of life, whatever the reason.

Since the last decade of the 20th century, and in particular since the first decade of the 21st century, a hypertextual digitization of data has been consolidating, as well as of the production and management of knowledge. While this predates the global COVID 19 pandemic, she significantly accelerated her naturalization. Introduction: An EU-funded project in five countries examined vulnerability mechanisms during the COVID-19 pandemic. The research team in Germany concentrated on people living at the intersection of migration and precariousness. The study aimed first to provide an understanding of how migrants living in precarious conditions in Munich had been affected by the pandemic, both from their own and from experts' perspectives. The second aim was to develop action recommendations to reduce structural vulnerabilities and increase resilience with a view towards improved pandemic preparedness. Methods: The study followed a two-phase process. The first was a qualitative study based on interviews with 25 migrants and 13 experts. In the second, researchers developed action recommendations based on the vulnerability/ resilience factors that had been generated in the first phase. Three consecutive meetings with stakeholders (expert panel, focus group discussion with two migrant organization, meeting with the Munich Migration Council) were then held to further strengthen the draft recommendations. Results: Content analysis revealed twelve vulnerability and eight resilience factors in three domains (COVID-19 prevention; human rights, living and housing environment; social support). Migrants had limited access to COVID-19 prevention measures; living conditions made outbreaks inevitable; uncertainty about legal status, employment, and housing, as well as stigma and discrimination, exacerbated their precariousness; social support had decreased; and resilience mechanisms had failed. The initial draft of recommendations contained 24 proposed actions. The meetings added recommendations such as enhancing psychosocial support, preventing ghettoization, improving social housing, preventing the interruption of language education in times of crisis, severe penalties for media stigmatisation and proactive truth-telling. The final list included 30 actions. Conclusion: In Munich, the COVID-19 pandemic exacerbated vulnerability mechanisms commonly associated with being a migrant. The recommendations developed here speak to those vulnerabilities but need to be refined further to be more actionable and comprehensive. Nonetheless, the recommendations and the processes that led to them highlight the importance of migrant-inclusive approaches and empowerment in increasing migrants' resilience to future crises.

INTRODUCTION: Cutaneous abscesses are a common complication of intravenous substance use. Although these skin and soft tissue infections represent one of the main causes of emergency room visits and hospitalizations in people who inject drugs (PWID), data on their prevalence and causes are scarce. The present study was part of the larger ANRS-OUTSIDER project and aimed to study the factors associated with skin abscesses in PWID, focusing in particular on the different stages of injection. METHOD: Analyses were based on data of the 164 persons who regularly injected psychoactive substances participating in the French ANRS-OUTSIDER project. A face-to-face questionnaire collected data on sociodemographics, substance use, injection practices, and experience of skin abscesses in the previous six months. Factors associated with skin abscesses were studied using a logistic regression model. RESULTS: Of the 140 participants for whom abscess data were recorded at inclusion, 35% reported having a skin abscess in the previous six months. Factors associated with skin abscesses were: benefiting from universal health coverage (PUMA/CMU/AME) (Odd ratio (OR) = 0.28, confidence interval (CI) à 95%=0.08-0.99), finding the vein to inject by touch (OR=3.44, 95% CI=1.41-8.43) and licking the needle before injection (OR=5.16, 95% CI=1.10-24.30). CONCLUSION: Our results highlight that skin abscesses were very frequent among the complications observed in our sample of French PWID, and that certain injection practices fostered their occurrence. These data will provide stakeholders with useful information to improve prevention and harm reduction messages for PWID.

RESUMO

The specialized therapy for skill acquisition model for children, especially autistic ones, is growing under neoliberal logic. Health professionals experience the precariousness of their work, with repercussions on the care of their health and that of children and families. To deepen the understanding of such aspects, this qualitative, documentary and retrospective research analyzed 131 reports of therapists on social media, under the conditions, dimensions and precariousness of work and historical dimensions, politics and contemporary challenges of care. The results show five themes about the precariousness of work, and three addressing violence and violation of children's rights, institutionalization and the precariousness of relationships. It discusses how the relationship between precarious work and assistance in the "autism industry" affects professionals, children and their families. In order to overcome historical institutionalizing behaviors and promote effective care, integrated with the children's daily experiences and demands, the urgency of the involvement of everyone who is interested in this issue is discussed in order to find dignified solutions in view of the seriousness of the problem at hand.


O modelo de terapias especializadas para aquisição de habilidades para crianças, especialmente autistas, está crescendo sob a lógica neoliberal. Profissionais da saúde vivenciam a precarização do seu trabalho, com repercussões no cuidado de sua saúde e das crianças e famílias. Para aprofundar a compreensão de tais aspectos, esta pesquisa qualitativa, documental e retrospectiva, analisou 131 relatos de terapeutas de uma mídia social, sob as condições, dimensões e precarização do trabalho e dimensões históricas, políticas e desafios contemporâneos da assistência. Os resultados evidenciam cinco temas sobre a precarização do trabalho, e três abordando violência e violação dos direitos das crianças, institucionalização e a precarização das relações. Discute-se como a relação entre trabalho precário e assistência na "indústria do autismo" afeta profissionais, crianças e suas famílias. Para superar condutas históricas institucionalizantes e promover um cuidado efetivo, integrado às vivências e demandas cotidianas das crianças, discute-se a urgência do envolvimento de todos a quem essa questão interessa para encontrar soluções dignas diante da gravidade do problema em pauta.

Even if nursing students have obtained social progress following different periods of mobilization and negotiations, we must admit that their situation remains worrying. Many of them live in precariousness and undergo very difficult training conditions. Instrumentalized, even mistreated, by caregivers who are at the end of their rope because they work in institutions that are in a critical situation, the students no longer learn and perform the tasks of the missing staff. The Covid-19 crisis is a striking example of this.

While insecurity can affect students in any field of study, those studying to become nurses are particularly badly off. Receiving lower internship allowances than other students, depending on the region and not on the regional center for university and school works, they also have to deal with a very busy training program. Many of them then turn to temporary work, which allows them to earn what they need to continue learning their future profession. This situation should no longer exist in 2023: all students should be able to train under good conditions.

BACKGROUND: Severe overcrowding of emergency departments (EDs) impacts the quality of healthcare. One factor of this overcrowding is precariousness, but it has rarely been considered a key factor in designing interventions to improve ED care. Health mediation (HM) aims to facilitate access to rights, prevention, and care for the most vulnerable persons and to raise awareness among healthcare providers about obstacles in accessing healthcare. We here present the results of an ancillary qualitative study to explore the prospects regarding a health mediation intervention implemented in EDs for deprived persons who are frequent ED users, from professionals' and patients' perspectives. METHODS: Design, data collection, and data analysis were done according to a psychosocial approach, based on thematic content analysis and semi-structured interviews of 16 frequent ED users and deprived patients exposed to HM and of 14 professionals in 4 EDs of South-eastern France. RESULTS: All patients reported multifactorial distress. Most of them expressed experiencing isolation and powerlessness, and lacking personal resources to cope with healthcare. They mentioned the use of ED as a way of quickly meeting a professional to respond to their suffering, and recognized the trustworthy alliance with health mediators (HMrs) as a means to put them back in a healthcare pathway. The presence of HMrs in EDs was appreciated by ED professionals because HMrs responded to requests they were not able to access and were perceived as an efficient support for caring for deprived persons in emergency contexts. CONCLUSIONS: Our results are in favour of health mediation in EDs as a promising solution, requested by patients and ED professionals, to cope with frequent ED users and deprived patients. Our results could also be used to adapt other strategies for the most vulnerable populations to reduce the frequency of ED readmissions. At the interface of the patients' health experience and the medico-social sector, HM could complete the immediate responses to medical needs given in EDs and contribute in alleviating the social inequalities of health.
